Magic Mountain Youth Event: Grades 3-7

Summer is over, but the fun never ends! Join your favorite Camp Wise staff for a full day of fun at Magic Mountain with your favorite counselors! You don’t want to miss out on the first Camp Wise Youth Event of the year! Limited spaces are available.

This event is only for current 3rd-7th graders. Please pack a lunch, snack, and water bottle, or bring a maximum of $40 for lunch, snacks, and souvenirs. Campers may arrive between 9:15-9:30 a.m. The bus will leave Stephen Wise Temple promptly at 10:00 a.m. on Saturday, February 1, and pick up is between 7:00 - 8:00 p.m. at Stephen Wise Temple. Students in grades 5-7 may stay overnight for a sleepover, and be picked up between 8:30 - 9:00 a.m. on Sunday, February 2.

Early Bird ends Friday, January 10, and registration closes on Wednesday, January 29 at 11:59 p.m. An email with details and what to bring will be sent on Thursday, January 30 to all registered families. For more information, contact Tatiana Soofer at (310) 889-2263 or email tsoofer@wisela.org.

Price: Early Bird $115, After January 10 $150

Cancellation Policy: No refunds after January 15. Event will occur, rain or shine.

Authorization to Consent to Treatment of Minor
Important: This permission slip is due the day of the event.

Your child(ren) may not attend the event without it. (I)(We), the undersigned, parent(s) of 

, a minor, do hereby authorize the Youth Director, or his authorized representative, as agent(s) for the undersigned, to consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital care which is rendered under the general or special supervision of any physician and/or surgeon licensed under the provision of the California Medicine Practice Act or the medical staff of a licensed hospital, whether such examination, diagnosis, or treatment is rendered at the office of said physician or at such hospital. It is understood that this authorization is given in advance of any specific examination, diagnosis, treatment, or hospital care being required and is given to provide authority and power on the part of the Youth Advisor, or his/her authorized representative, to give specific consent to any and all such examinations, diagnoses, treatment or hospital care which the aforementioned physician in the exercise of his/her best judgment may deem advisable. This authorization is given pursuant to the provisions of section 25.8 of the Civil Code of California. This authorization shall remain effective until the end of the current youth group event. 

No refunds after October 18, 2019.  Any cancellations made before then must be sent in writing to TSoofer@WiseLA.org.
